Dear Loyola Parents:
On behalf of the Department of Athletics and Wellness, I would like to welcome you and your family to the 2015-16 academic year at Loyola University New Orleans.
Please carefully read the Loyola University New Orleans Student-Athlete Handbook. The handbook contains vital information and expectations regarding department standards, policies and procedures that can enhance the student athlete’s experience at Loyola New Orleans. All student athletes are required to read the information.
Several forms must be completed by the student athletes before any student athlete can compete in practice or games. Initially, our student athletes are required to complete the “Emergency/Medical /Insurance Information” form. The completed EMI form along with a back and front copy of the current insurance card must be received in our office, prior to the first practice. These documents can be hand delivered, faxed, or mailed to our Athletics office to the attention of our office manager, Anastasia E. Ochoa.

Loyola provides secondary medical insurance coverage for our student athletes.
Take a few moments to closely review the medical insurance coverage section in the Student Athlete Handbook. This section highlights:
- what bills are covered (Athletic vs. Non-athletic)
- who is responsible for “deductibles”
- charges above the “reasonable and customary” fee
We ask that any correspondence you receive from your healthcare insurance provider regarding payment(s) be sent to Mr. Turnage within 30 days to the Athletics office. Also, to prevent delayed claims or claim denials, please notify us immediately of any changes with your health insurance provider. You will also want to confirm with your primary insurance provider that the student athlete will be covered in each of the 50 States. International students are required to have a Global Medical Insurance Policy.
Mr. Buddy Turnage (rturnage@loyno.edu) serves as our insurance liaison for our department. Please feel free to contact him regarding any questions or concerns about your son/daughter’s medical insurance.
